There are two forms that we use at PDY to protect both the students and the church.
These forms are mandatory for every special event hosted by PDY.
These forms may be downloaded below in either Word (.doc) or Adobe (.pdf) format.
The forms are as follows:
Medical Information Form.
A new medical information form is required every 4 years.
Event Release.
This form serves as a medical & liability release as well as a permission form.
A new Event Form is required for every event.
